1. Understanding the Service

I understand that permanent jewelry is custom-sized jewelry joined using a specialized welding process. The jewelry does not have a traditional clasp and is intended for continuous wear.

I understand that welding equipment is used during the application process and that Synergy Jewelry LLC will take reasonable precautions to protect my skin.

2. Risks & Sensitivities

I understand that potential risks may include temporary discomfort, skin irritation, allergic reactions, minor burns, or injury if the jewelry becomes caught, pulled, or tangled.

Synergy Jewelry LLC uses gold-filled and sterling silver jewelry. I understand that individual sensitivities or allergic reactions may still occur.

I agree to disclose relevant safety concerns before receiving my jewelry. Synergy Jewelry LLC may decline or postpone a service if it cannot be performed safely.

3. Jewelry Care & Removal

I understand that permanent jewelry is not indestructible. Chains may stretch, break, or become damaged through normal wear, accidental pulling, snagging, or other circumstances.

I acknowledge that I have been given an opportunity to ask questions about proper jewelry care.

I understand that permanent jewelry can be removed by carefully cutting the chain with an appropriate tool.

If jewelry removal is necessary for medical care, imaging, an emergency, or another safety reason, I agree to follow the instructions of the relevant healthcare professional and not delay necessary care.

4. Sales, Repairs & Weld Guarantee

I understand that permanent jewelry purchases are final sale, subject to applicable law.

Synergy Jewelry LLC guarantees its weld workmanship but does not guarantee that chains or other jewelry components will never stretch, break, or become damaged.

If an issue occurs within the first 14 days after application, Synergy Jewelry LLC offers a complimentary repair or replacement at an upcoming Synergy pop-up, subject to evaluation of the jewelry and its condition.

Damage unrelated to weld workmanship is not covered by the weld guarantee. Services outside complimentary coverage may be subject to additional charges.

5. Voluntary Participation & Assumption of Risk

I voluntarily consent to receive permanent jewelry from Synergy Jewelry LLC. I acknowledge that I understand the nature of the service and its reasonably foreseeable risks.

I have had an opportunity to ask questions, and I voluntarily assume the ordinary risks associated with the service and wearing the jewelry.

To the extent permitted by applicable law, I release Synergy Jewelry LLC and its owners, employees, and representatives from claims arising from the ordinary risks of the service, including claims based on ordinary negligence.

This release does not apply to gross negligence, reckless conduct, intentional misconduct, or claims that cannot legally be waived.

6. Consent & Acknowledgment

I confirm that the information provided is accurate to the best of my knowledge. I have read and understand this document, have had an opportunity to ask questions, and voluntarily agree to its terms.
